ZD3 L/R Phase (or Timing?) Issues

My ZD3 is acting a little oddly. It functions perfectly the majority of the time but occasionally the left and right channels will sound as though they have been pulled apart sideways by about 10 feet. This will happen roughly every 15-20 minutes. I haven't timed the interval but I plan to take a video of the system while it's playing in order to see if the issue happens on a regular interval or not.

My suspicion is that there is an issue with some clock signal internal to the DAC that is causing a phase imbalance or time delay between the two channels. This happens across every input and it does not seem to matter which analog output is being used; balanced or single ended. I have narrowed the issue down to the ZD3 by changing out amplifiers, interconnects, and sources.

My next troubleshooting steps will be to test the ZD3 with another 12VDC PSU - though I don't think this is the issue - and to try changing the operational amplifiers as I have the launch edition ZD3 that was supplied with upgraded op-amps. I will update this post with the results later this week.

Has anyone else experienced an issue like this? Otherwise I love this DAC for its feature set (HDMI ARC + CEC!!) and impeccable measurements.
Thanks in advance!
